Up to 6 times your salary with a limit of $500,000 in $10,000 increments
You may elect coverage for your spouse for up to 50% of the employee’s paid amount in $5,000 increments.
You may elect up to $10,000 in increments of $2,000 worth of coverage for your dependent children as well.
