Elect an employee life amount 1x-6x annual earnings to a maximum of $1,000,000
You may elect coverage for your spouse to a maximum of $30,000 not to exceed 50% of the employee's coverage amount.
You may elect up to $10,000 worth of coverage for your dependent children as well.
