$0 copay for eye exam or eyeglass lenses
Retail frame allowance up to $130 or up to $180 at Visionworks locations for Low Plan
Retail frame allowance up to $200 or up to $250 at Visionworks locations for High Plan
Eye Exam, Lenses, Frame, and contact lens evaluation available once every 12 months
